Applicants will be required to submit the following:
• Bio data.
• Significant contribution(s) made towards the development of the disability community
• Supporting reference(s) / evidence(s) such as pictures, videos, audios, news publications, etc.
• A minimum of two (2) referees and their contact details
Significant Contributors in the areas of:
• Disability Sports and Disability Entertainment
• Entrepreneurship
• Education
